'Profile Video' Application

Yes I'd like to apply for a subsidised $550 Profile Video captured by Jason. I understand that 10 (only) subsidised packages will be offered and Jason reserves the right to choose the successful applicants based on their individual story needs & merit. I also enter this application on the understanding that, if successful:

• My interview would be conducted sometime from mid June to mid July 2023

• I can be available for a 30min zoom call briefing with Jason

• I can be available for between 2 and 3 hours for Jason to film my interview

• I understand only 1 x revision round will be allowed on the edited video

• My interview will be edited into a 3 minute video which I can use for my own promotion

• Jason will use these interview shoots to create additional training resources same time

• I agree to give permission for my content to be used in the training resource production

• Applicants will only be selected from the Central Coast, Newcastle & Sydney areas

• Payment will need to be made in full prior to the agreed filming date

• Notification of successful applicants will take place by June 14, 2023
